  1. ব্যাক্তির নাম (বাংলা-ইংরেজি উভয় ভাষায়), পিতা-মাতার নাম, জন্মতারিখ ও জাতীয় পরিচয় নিবন্ধন নম্বর।
  2. কার্ডের পেছনে ব্যাক্তি যে এলাকা থেকে ভোট দিচ্ছেন তার ঠিকানা, রক্তের গ্রুপ ও জন্মস্থান্ উল্লেখ করা থাকবে।
  3. স্মার্ট কার্ডের চিপ (তথ্যভান্ডার) এ থাকবে ৩২ ধরনের তথ্য, যা মেশিনে পাঠযোগ্য।
  4. এবারের স্মার্ট কার্ডের সাথে পূর্বের কগজ দ্বারা ল্যামিনেট করা কার্ডের বেশ পার্থক্য রয়েছে। এবারের কার্ডটি প্লাস্টিকের (পলিমার) তৈরি, তাই এটি হবে মজবুত ও দীর্ঘস্থায়ী।
  5. কার্ডটির মেয়াদ করা হয়েছে ১০ বছর পর্যন্ত।
  6. নারীদের ক্ষেত্রে পূর্বের কার্ডে স্বামীর নাম ছিল, পিতার নাম ছিল না। অপর দিকে পুরুষদের ক্ষেত্রে কার্ডে স্ত্রীর নাম ছিল না, যা বৈষম্য মূলক বলা হচ্ছিল। এবার নারীদের স্মার্ট কার্ডে স্বামীর নামের পরিবর্তে থাকছে পিতার নাম।
  7. কার্ডের চিপ-এ ব্যাক্তির পেশা, স্থায়ী ঠিকানা, বর্তমান ঠিকানা, বয়স, বৈবাহিক অবস্থা, জন্মনিবন্ধন নম্বর, শিক্ষাগত যোগ্যতা, সনাক্তকরণ চিহ্ন, ধর্ম, পাসপোর্ট নম্বর, আয়কর সনদ নম্বর, ড্রাইভিং লাইসেন্স নম্বর, লিঙ্গ, শিক্ষাগত যোগ্যতা, টেলিফোন ও মোবাইল নম্বর, মা-বাবা ও স্বামী বা স্ত্রীর মৃতুসংক্রান্ত তথ্য, প্রতিবন্ধি বা অসমর্থতার তথ্য রয়েছে।
  8. স্মার্ট কার্ডের কভারে রয়েছে, জাতীয় ফুল, জাতীয় পাখি, জাতীয় স্মৃতিসৌধ ও চা-পাতা, মুক্তিযুদ্ধকালীন ও বর্তমান জাতীয় পতাকা, জাতীয় সংগীত ইত্যাদি।
  9. দেশের ৯ কোটি ভোটার কে স্মার্ট কার্ড বিতরণের এই প্রকল্প গ্রহন করা হয় ২০১১ সালে, এজন্য বিশ্বব্যাংক থেকে ঋন নিয়ে ৮০০ কোটি টাকা ব্যায় করা হয়েছে।

স্মার্ট কার্ড কবে পাবো, এই প্রশ্ন এখন সবার। প্রাথমিক পর্যায়ে স্মার্ট কার্ড পাচ্ছেন ঢাকা উত্তরের ১ নং ওয়ার্ড(উত্তরা) এবং ঢাকা দক্ষিনের ১৯,২০ ও ২১ নং ওয়ার্ড (রমনা থানা)-এর সম্মানিত ভোটার রা। তাদের ভেতরে স্মার্ট কার্ড বিতরণ করা হবে ৩ থেকে ২৬ অক্টোবরের মধ্যবর্তী সময়ে। যেকোন ভোটার তাদের ফোনে ১০৫ ডায়াল করে জানতে পারবেন তাঁর স্মার্ট কার্ড সংক্রান্ত তথ্য।

শুরুতে যদিও নির্বাচন কমিশন উদ্যোগ নিয়েছিল সব ওয়ার্ডে একযোগে স্মার্ট কার্ড বিতরন করার, কিন্তু প্রস্তুতিমূলক কাজ শেষ না করতে পারায় দুই সিটির চার ওয়ার্ডে এবং কুড়িগ্রামের দাশিয়ারছড়ায় স্মার্ট কার্ড বিতরণের সিদ্ধান্ত নেয়া হয়েছে বলে জানা গিয়েছে।

স্মার্ট কার্ড সংগ্রহের সময়ে ব্যাক্তির হাতের সব আঙ্গুলের ছাপ ও চোখের আইরিশ স্ক্যান করা হবে, এবং কাছে বিদ্যমান কাগজের তৈরি কার্ডটি জমা দিতে হবে।

জাতীয় পরিচয়পত্রের স্মার্ট কার্ড পেতে নাগরিকদের পুরনো কার্ড জমা দেওয়ার পাশাপাশি ১০ আঙুলের ছাপ ও চোখের মণির ছবি দিতে হবে। এক্ষেত্রে কারও কার্ড হারিয়ে গেলে প্রথমে পুরনো কার্ডটি তুলে তা জমা দিয়ে স্মার্ট কাড নিতে হবে।

সিটি করপোরেশন ছাড়া দেশের অন্যান্য এলাকায় পৌরসভা কার্যালয় ও ইউনিয়ন পরিষদ থেকে ভোটারদের মাঝে স্মার্ট কার্ড বিতরন কার্ড দেওয়া হবে বলে ইসি সচিব জানিয়েছেন।

স্মার্ট কার্ড প্রস্তুত ও বিতরণের লক্ষ্যে ফ্রান্সের অবার্থুর টেকনোলজিস সংস্থার সঙ্গে চুক্তি অনুযায়ী সব ভোটারের হাতে স্মার্ট কার্ড পৌঁছাবে ২০১৬ সালের মধ্যে।
